Project Overview:

The goal of the eLTSS project is to develop an integrated FHIR-enabled system, allowing HIEs and care providers to seamlessly access LTSS patient CarePlan-related data using FHIR APIs, while adhering to FHIR security standards.

Our dedicated team, comprising project managers, interoperability leaders, FHIR Architects, FHIR subject matter experts (SMEs), FHIR API developers, testers, and database developers, worked collaboratively to achieve this milestone. By leveraging the robust capabilities of the eLTSS FHIR IG, we have enabled seamless, bidirectional data exchange, significantly enhancing care coordination for long-term care patients.

    The project’s success is highlighted by the ability of multiple Health Information Exchanges (HIEs) to connect seamlessly and access patient CarePlan data in a bidirectional manner. Impact on Long-Term Care:

Long-term services and supports (LTSS) assist millions of individuals with disabilities and chronic illnesses, helping with activities like eating, dressing, and managing money. These services are provided in various settings - Services provided at home or in community settings are known as Community-Based Long-Term Services and Supports (CB-LTSS) or Home and Community-Based Services (HCBS).

Enabling electronic data exchange for patient CarePlans for LTSS patients among multiple care facilities and care providers significantly impacts real patient care.

The 56 core data elements included in the eLTSS Dataset, defined by CMS and ONC, are implemented for data exchange using FHIR R4, US Core, and eLTSS FHIR profiles.
