Job Summary:
We are seeking an Epic Healthy Planet Analyst with experience in population health management, care coordination, and Epic Healthy Planet application support. The ideal candidate will be responsible for system configuration, optimization, reporting, and workflow improvements to support population health initiatives.
Key Responsibilities:
- Configure, maintain, and optimize Epic Healthy Planet workflows and functionality.
- Gather and analyze business requirements related to population health and care management.
- Support system implementations, upgrades, testing, and issue resolution.
- Develop and maintain registries, dashboards, measures, and reporting solutions.
- Collaborate with clinical, operational, and technical teams to improve patient outcomes and care coordination.
Required Skills:
- Experience with Epic Healthy Planet application support and configuration.
- Strong understanding of population health management and value-based care programs.
- Experience with workflow analysis, testing, and production support.
- Strong analytical, communication, and problem-solving skills.
- Epic Healthy Planet certification preferred.
