Job Title: Clinical Post-Acute Care (PAC) SME
Duration: 5 Months
Travel: Up to 50% (Wisconsin)
Role Overview
Seeking a Clinical PAC SME to support post-acute care strategy, implementation, and optimization within inpatient case management initiatives. This is a client-facing role focused on improving care delivery, utilization, and financial outcomes through hands-on advisory and program leadership.
Key Responsibilities
- Lead design, implementation, and optimization of Post-Acute Care programs
- Provide program and engagement management across multiple workstreams
- Partner with client stakeholders, clinicians, and senior leadership to drive outcomes
- Support initiatives to improve care efficiency, post-acute utilization, and cost performance
- Deliver high-quality presentations, insights, and recommendations
Required Qualifications
- 5+ years in healthcare consulting, hospital operations, or project management
- Strong analytical, problem-solving, and organizational skills
- Proficiency in MS Excel and PowerPoint
- Proven ability to manage multiple workstreams in fast-paced environments
- Strong communication and stakeholder management skills, including senior leadership engagement
Preferred Qualifications
- Experience in post-acute care or clinical transformation initiatives
- Licensed Nursing Home Administrator (LNHA) preferred
- Experience with change management and program delivery across complex organizations
